The initial collection of your personal information is needed for either the first-time new client or to update information from existing clients to provide my full Medicare Insurance Broker services. Information from this form is also used to generate a Medicare Plans Quote.

Scope of Sales Appointment (SOA)

Under Medicare rules, an SOA is required before an agent-broker can discuss or review specific Medicare Advantage plans. A waiting period of 48 hours is also required.
